How do I upload my declaration page from my professional liability policy?

If you need to upload a copy of the declaration page from your professional liability policy, you can accomplish this through the Attorney Portal.

There are two ways to access the Attorney Portal:

1. Direct Link

You may go directly to the portal using the following link: https://attorney.uslegalservices.net/login

2. Through the Website

Visit the main website at:  https://uslegalservices.net/

If you are using the website, the link to get to the portals is in the top navigation bar.

1. In the top right corner of the navigation bar, click the Log In button.

2. You will be taken to the portal landing page.

3. Click the Go to Attorney Portal button.

4. You will then be redirected to the Attorney Portal.

Once you have logged into the attorney portal, click Profile in the top navigation bar.

In the main profile page, click Practice Details in the left navigation panel.

1. Click the Edit Details button.

   

2. A pop-up screen will show.

   
   

3. Scroll down to the ATTORNEY FILES section.

4. Select W-9 from the file type dropdown.

   
   

5. Click the Choose File button.

6. This will open a file browser window on your computer. Navigate to the W-9 file you wish to upload, highlight it, then click Open.

    

   

7. The name will appear in the Choose File field.

8. Click the Upload File button.

   

9. You will get a pop-up once the file is successfully uploaded.

    

   

10. The file will appear with all other uploaded files in the main Practice Details screen.
